SMMC Visiting Nurses Ranked Among the Top 25 Percent
of Home Health Providers in the Country
Kennebunk, Maine – SMMC Visiting Nurses today announced that it has been named to the 2008 HomeCare Elite, a compilation of the most successful Medicare-certified home health care providers in the United States. This annual review identifies the top 25 percent of agencies, ranked by an analysis of performance measures in quality outcomes, quality improvement and financial performance.
“The 2008 HomeCare Elite winners exemplify a commitment to providing their patients with optimum care while performing at the highest level,” said Nancy Buller, Senior Director of Marketing Communications at OCS, Inc. ‘We congratulate SMMC Visiting Nurses on being one of the Top home care agencies in the country.”
SMMC Visiting Nurses Executive Director, Elaine Brady, credits the significant decrease in the number of patient readmissions to the hospital and emergency room visits as two indicators which enabled the organization to rank as one of the HomeCare Elite. She said, “Our dedicated staff and managers have been working hard to address patient needs early in our admission process and have focused on helping patients correctly understand their medications. We have also strived to incorporate telehealth monitoring (daily checks on vital signs) through our Well@Home Program into the care of patients who might be at risk for setbacks. This is a great way to kick off our centennial year celebration- 1909 to 2009!”
The HomeCare Elite is the only performance recognition of its kind in the home health industry. The complete list of the 2008 ranked agencies can be viewed here: HomeCare Elite, 2008 .
